I don't think there is any way to print one from FitDay; however, a quick Google search finds many available, such as this one:

One could also whip up a quick one in Word or Excel, if one so desired. I used to carry a Day-Timer which would also be good, or even a small memo pad. If you've a smart phone, I'm sure "there's an app for that" as well.
