Request and RMA Number
To receive a Return Merchandise Authorization ("RMA") number
complete the following request form.
If we are able to identify your order,
and the request is made within the timeframe as discussed in our return policy,
you will be sent a RMA number by email.
You must provide sufficient information for us to be able to identify your transaction.
You must enter your name and email.
If you have the order number for your purchase, enter it.
Otherwise, use the Other Information field
to provide us with any necessary information to identify your transaction,
such as the date of transaction and/or items purchased for your order.
